 | Consent Form: At long last and after nearly a year in the making , the
Oklahoma State Department of Health has approved a new Certificate of Request for
Anatomical Gift. This form takes the place of any other form from the state and has been
approved for use by ALL agencies involved in organ and tissue recovery. Your hospital may
obtain the form by calling the Oklahoma State Department of Health, Special Services
Division at 405-271-6576 and requesting Form ODH 942. |
